The trend of minimally invasive spine surgeries is on the rise. Keyhole surgery offers the advantages of minimal soft tissue damage, lesser blood loss, lesser postoperative pain, and lesser hospital stays. A paramedian approach preserves the posterior tension bend, thus avoiding spinal instability. The posteriorly and laterally located intradural extramedullary tumors spanning over one or two spinal segments can be excised safely using tubular retractors. This artistic illustration demonstrates the keyhole concept for the excision of an intradural extramedullary schwannoma. The first image shows an axial section demonstrating the approach, and the second image shows a microscopic view through the tubular retractor (Figure). The principles of surgery essentially remain the same as conventional microsurgery.
