Conventional approaches to Meckel's cave include middle fossa transcranial approaches through craniozygomatic osteotomies either through extra or intradural exposures. These approaches involve retraction of the temporal lobes with associated complications. Although well described, the endoscopic trans pterygoid approach has selective indications. Extension of the tumor from Meckel's cave into the sphenoid sinus and infratemporal fossa was one of the common indications. The disease extension from Meckel's cave to the posterior fossa is technically challenging compared to tumors localized to the middle fossa. The advantage in the trans pterygoid approach is the access to various areas like the foramen of ovale, vidian canal and Meckel's cave, foramen Lacerum, petrous ICA, and fossa of rossenmuller.
