Many thanks for forwarding the comments on the article titled 'Non- motor symptoms in an Indian cohort of Parkinson's disease patients and correlation of progression of non motor symptoms with motor worsening' published in Neurology India. [1]

Unified Parkinson's disease scale is one of the scales we use to determine the progression of disease. Higher UPDRS scores indicate advanced state of the disease. Part III of this scale includes motor examination. The purpose of our study was to determine the occurrence of non motor symptoms with advancing UPDRS motor scores. The terminology 'progression' in the published article was used in this context. Non motor features such as loss of smell, constipation, depression and anxiety are noted in early stages of Parkinson's disease as well as in the premotor stage of Parkinson's disease. Cognitive decline and dementia are noted with progression of the disease. The purpose of the study was compare higher or lower UPDRS motor scores and the associated presence or absence of non motor symptoms at that individual score.

The control subjects were included only to decide whether or not these non motor symptoms are disease specific or may be seen as normal ageing symptoms.
